This notice of change in mailing or email address is only required if you’ve made changes since filing your initial divorce packet. If nothing has changed, don’t complete it; save it for future use. If there are changes, you’re confirming your previous and current email addresses. You must also certify that all parties, including the clerk of court, have received a copy and note how they received it. If you’ve opted for the e-portal system, just sign and upload it for automatic notification. Otherwise, file the original with the clerk of court and send a copy to the other party.
